Before attending your appointment, please fill out the following form:

Pre-travel Health assessment form

Level One

  • A review of Immunization history, health history, and current medications

  • Recommendations for immunizations/medications for your trip if required based on risk, health history, medications and itinerary

  • Provide education regarding health and travel safety in relation to destinations

  • Administer all vaccines purchased during appointment – Injection fee not included

  • Inform public health of updated immunizations

Level Two (includes malaria risk)

All of level 1 services plus:

  • A review of malaria risk

  • Personalized recommendation of malaria preventative medication if indicated

  • Education on Malaria and prevention strategies

  • Provide prescription for anti-malarial medication as required

Level Three (includes malaria and yellow fever risk)

All of level 1 and 2 services plus:

  • A review of yellow fever risk

  • Provision of yellow fever vaccination

  • Provision of International Certificate of Vaccination or Prophylaxis (ICVP)

  • Education on Yellow Fever and prevention strategies
